Becoming A Patient

Neighborhood Health makes it easy to get the care that you need.

Who Qualifies:  Anyone can come to a Neighborhood Health center for care. You can be insured or uninsured. You do not have to live in Nashville. We care for US Citizens and new immigrants and refugees.


Appointments:  Making an appointment is easy

  • Go to the Locations page if you want to pick a health center nearest to you.
  • Call 615.227.3000
  • Neighborhood Health centers accept appoinments from 8:00 am to 5:00 pm, Monday through Friday.
  • Neighborhood Health at Madison is open until 10:00 pm, Monday - Friday, and Saturday until 4:00 pm for appointments.

To make an appointment, have the following information ready:

  • Your name and address
  • The reason for your visit
  • Health Insurance Information if you have insurance. Insurance is not necessary.
  • Language: If you need an interpreter to help with the visit, let them know your preferred language when you call.

Same Day Appointments:

We have openings each day for medical care and dental care. If you need to see a medical provider or a dentist that day, you can walk in and let customer service know. They will work you in as soon as possible. To make sure you are seen quickly, call ahead first and make an appointment, 615.227.3000.

What to Bring


When you are coming to a Neighborhood Health Center for the first time, please bring the following:

  • Photo ID: any card you have (driver's license, work ID, other)
  • Your medications and supplements like vitamins
  • Your pharmacy name and location
  • Your medical history: any information about surgeries, family health history, immunization record, or any other helpful information.
  • $25.00: If you are insured you will probably have a co-pay. If you are not insured and qualify for a discount fee, $25.00 may cover that fee.

If you have insurance please bring

  • your Health Insurance Card
  • Co-payment if you have one

Discount Program for those without insurance:

  • Neighborhood Health has a Discounted Fee Program for those who don't have insurance.  If you qualify, we will reduce the amount you are asked to pay based on your income and the size of your family.

To qualify for a discount, please bring:

  • Proof of income: Pay Stub, W2, bank statement, most recent tax return, Social Security letter, letter from your employer or letter from a church or agency who knows your financial circumstances.

What if I don't have $25.00:

  • If you don't have $25 when you visit, bring any amount that you can pay toward your visit. You will not be denied care. You will be billed.

Your First Visit

Please arrive 15 minutes before your appointment so that you can complete registration.

 

Registration:  When you arrive, Customer Service will ask you to fill out a form. You can save time by printing a copy and filling it out before you come to the center.

You will also be asked to complete a health history form. This will help the provider make a decision about your care.


Exam:  A Medical Assistant will call your name and you will be taken to an exam room. The MA will also check your height, weight, temperature and blood pressure. You may be asked to complete other health surveys. This will help us know the services you need. Then you will meet with your doctor, nurse practitioner, dentist or counselor.


Lab Test:  The provider may want you to have some lab tests. They will order these tests and you may be asked to provide a urine sample or to have blood drawn. Or you may be asked to stay for an EKG which is a way to study your heart. The samples will then be sent to a lab and Neighborhood Health will receive the result. You will be contacted if the result requires further visits.


Shots:  If you need a shot or your child needs an immunization, this will be done at the end of your visit.


Referrals:  Because of your health, you may need to visit a specialist, get an x-ray, have an ultrasound or have a test. The staff will let you know and they will make an appointment for you and contact you about the time of that appointment.  Please keep that appointment.


Hospital:  We know that if you need to be hospitalized, where you go is very important to you. Neighborhood Health works with all hospitals in Nashville and middle Tennessee. If you have insurance, we will refer you to the hospital of your choice where they accept your insurance. If you do not have insurance, Neighborhood Health works with Nashville General where there is also a discounted fee program for residents of Nashville.


Prescriptions:  Generally your prescription will be sent electronically to the pharmacy of your choice and you can go to the pharmacy and pick it up.


Check-out:  When you check out, you will receive a summary of your visit. If you don't understand how to take your medication or have any other questions, please ask.

You may find that due to the testing and other care you may have a slight extra charge. You will be billed for this. Neighborhood Health will work with you if you need a payment plan.  We do not use a collection agency.

 

Payment Options

Neighborhood Health will help you get the most affordable care possible. We accept the following types of payment and offer the following assistance:

  • TennCare:  United Healthcare, AmeriGroup, Cover Kids and Blue Cross/Blue Shield
  • Most Private/Commercial Insurance and Managed Care Plans
  • Medicare
  • For those without insurance: Discounted Fee based on income and family size
  • Cash, Check or Credit/Debit card
  • Pregnant?  You can have insurance Coverage. A Counselor will help you get enrolled.
  • We can help you enroll in health insurance coverage through the Insurance Exchange. Let us know if you would like to speak with a counselor.
